US Rowing Waiver – The USRowing Waiver will be emailed upon online completion in September after Fall registration.
FSRA Waiver – The FSRA Waiver informationwill be emailed upon online completion in September after Fall registration.
The Registration Packet should be printed and completed per the instructions and returned directly to the Boathouse, or at the Google Classroom Link below.
High School Athletes - Due by Aug. 31st
Middle School Athletes - Due by Sept 4th or 1st practice attended.
Please DO NOT return any team paperwork to the school athletic office. Please turn in all forms to your Coach.
Included in this packet is are the following necessary forms:
- Medical Release: MUST be notarized! This form gives permission for EMT's to treat your athlete in an emergency.
- Social Media Hazing Policy
- Behavior Policy
- City of Tampa Swim Test Waiver
- Team Volunteer Policy Acknowledgment
- EL2 Sports Physical
If you only need certain forms/ pages, please use your printer settings to print only those pages.
You can submit all papers either in person at the boathouse or at the Google Classroom Link Below:
